Removes the 'Child Container' idea (that was bad and would consume lots of mem) and instead we use Scopes. Ensures that the app event handlers are creating during a single Scope operation in the CoreBootManager. Changes MigrationBase and all sub-classes to have an IMigrationContext as a ctor param. Updates the MigrationResolver to resolve Migration instances with the passed in IMigrationContext value. Simplifies MigrationBase since now we know we have a context to work with, there's no need to GetUp/GetDown Expression methods. Updates the MigrationRunner to use the MigrationContext instead of creating one itself.

Cleans up the usages of auth cookies. OWIN is in charge of auth cookies but because we have Webforms, WebApi, MVC and OWIN, they all like to deal with cookies differently. OWIN should still be solely in charge of the auth cookies, so the auth extensions are cleaned up, the renewal now works by queuing the renewal and we have custom middleware detect if a force renewal has been queued and we renew the auth cookie there. Have obsoleted a few methods that should not be used that write auth tickets directly (this is purely for backwards compat with webforms). All of these changes now ensure that the auth cookie is renewed consistently between Webforms, WebApi, MVC and OWIN. Some changes also include ensuring that OWIN is used to sign out.

Got more of the sorting stuff working, started implementing much of the back office security checks in the content controller, updates some of the c# unit tests and got a self-hosted test running (but commented out currently until we can mock things properly).

unit tests for UmbracoModule, refactored the Umbraco.Web.Routing.Domains to not have static methods and created an
interface for it so that we can unit test it. Changed DocumentRequest stuff to internal. Finally got unit test working for the module

in our v4 ones.
